Methods: This was initiated with patient partners, medical students and clinical tutors. The central idea is a consultation between students and patient partners within each OSCE rotation, followed by a systematic debriefing session, thereby making the OSCE rotations formative. Results: This initiative has been institutionalised since 2022 in a medical school as a compulsory formative assessment at the end of the general practice clinical rotation. Discussion: It enables a rethinking of the role of these OSCE within the learning process offered to postgraduate medical students by integrating a progressive approach specific to pedagogical processes into the competency-based approach. Conclusion: Both students and supervisors, doctors and patients alike, perceive the benefits, which invites reflection on possible adjustments to this reform of postgraduate medical education. Méthodologie :Celui-ci a été initié avec des patients partenaires, des étudiants en médecine et des médecins enseignants L’idée maîtresse est une consultation entre étudiants et patients partenaires au sein de chaque station d'Examen Clinique Observable Structuré (ECOS) avec un temps de debriefing systématique et donc d’ECOS formatifs. Résultat : Cette initiative est institutionnalisée depuis 2022 dans une faculté de médecine comme examen obligatoire formatif de fin de stage de médecine générale. Discussion : elle permet de repenser la place de ces ECOS au cours du processus d’apprentissage proposé aux étudiants en médecine de 2ème cycle en y intégrant une progressivité propre aux processus pédagogiques dans l’approche par compétences. Conclusion : Tant les étudiants que les encadrants, médecins comme patients, y perçoivent les bénéfices, ce qui invite à réfléchir à de possibles ajustements dans cette réforme des études de 2ème cycle. 21ème siècle, propose donc, tel qu’indiqué dans l’édito, propose un dossier sur la pédagogie médicale et en sciences de la santé. Un dossier international francophone et intercontinental deux pays sont présent, la France et le Canada à travers la province du Québec. Une province souvent référencée comme « belle province » au sein de laquelle a été mise en forme dans le cadre d’un partenariat déjà franco-québécois, ou québeco-français, au choix, qui a permis la conceptualisation d’une des approches les plus adoptée à l’international, celle qui donne lieu au titre même de la revue. Le premier article présenté est justement proposé par une équipe de l’Université de Montréal qui a donné lieu à cette approche nommée initialement par leurs voisin états-uniens, « The Montreal model ».
